Licensing Model: Health Plan Licensing Royalty Model (“Health Plan Model”)
A minimum royalty of $100.00 applies for each annual release of the CPT Standard Data File in Covered Products licensed under the Health Plan Model. The minimum royalty of $100.00 shall be due to the AMA only if calculated per-Member royalties for the current annual release in all Health Plan Model Covered Products under the Agreement are less than $100.00.

Licensing Model: Laboratory Model
A minimum royalty of $100.00 applies for each annual release of the CPT Standard Data File in Covered Products licensed under the Laboratory Model. The minimum royalty of $100.00 shall be due to the AMA only if calculated per-Laboratory Test royalties for an annual release are less than $100.00.
